Provider Monthly Reports

1. When is the Provider Monthly Report available?
The report is available on the website, www.emergencyscholarships.org on the 1st of each month. The report is only available for five business days. Please note that weekends and holidays are not included in this five day window.

2. When is the Provider Monthly Report due?
The report is due by the fifth business day of each month. After the fifth business day, the report is no longer available on the website.

3. How do I submit the Provider Monthly Report?
Please fax your completed report to 602.240.6326. You may also email to emergencyscholarships@vsuw.org or mail to 1515 E. Osborn Rd, Phoenix AZ 85014. Please note we do not recommend mailing it to us as it must be received by the fifth business day. Fax or email are the preferred methods for submission.

4. What does the Provider Monthly Report consist of?
If your center or home is participating in the Emergency Scholarship Program, you will be required to submit a monthly report containing information regarding the number of children by age category who were enrolled in the scholarship program for that month. Providers must indicate the number of days each eligible child attended in addition to a maximum of five scheduled absences. Please indicate the dollar amount that would have been collected for that child if there was no scholarship in place.

5. What happens if I don’t send in my Monthly Report?
Providers who do not submit their Provider Monthly Report by the fifth business day of the month will experience a delay in receiving reimbursement for the care they provided and may jeopardize their continued participation in the program.

6. If I have questions about the Provider Monthly Report, who should I call?
If you need assistance filling out the report or have a question regarding the information in the report, please call 602.631.4888.

Scholarship Payments

7. When will I receive my payment?
You will receive reimbursement for the care you provided in the previous month by the end of the month in which you submitted your Provider Monthly Report. Please note that there is no specific date that payments will be processed as the deadline for submitting the report changes each month.

8. If I have questions about my payment, who should I call?
Please call 602.631.4888 if you have questions regarding your recent Scholarship payment or reconciliation.

9. What if the scholarship reimbursement rate is less than my usual rate?
If the scholarship amount is less than the usual rate at your home or center, you may request the families using the scholarships to pay the difference between the two rates. The amount of family out-of-pocket costs to parents must be reported on Provider Monthly Report each month. Participating providers are required to waive all registration and other fees for scholarship recipients, with the exception of enrichment activities (i.e., music lessons or dance lessons).

10. What if the scholarship reimbursement rate exceeds my usual rate?
You will not receive scholarship payments that exceed your usual rate.

11. Can a scholarship be used for a 5 year old child's after school care?
No, according to Arizona statute, scholarships are only available for child care for children birth to age five who are not, and have never been, in kindergarten.
